The modem in system logs in as 0af0:d001. I use Kubuntu 11.10.
These are settings for usb_modeswitch which works for me:
################################################## #####
# Option HSO device
DefaultVendor =0x0af0
DefaultProduct =0xd001
TargetVendor =0x0af0
TargetProduct =0xd157
TargetClass =0xff
Interface =0x01
MessageEndpoint =0x02
ResponseEndpoint =0x89
MessageContent ="55534243785634120100000080000

CheckSuccess =4
I found it here: http://ubuntuforums.org/showthread.php?t=1351612&page=3 from post which has been written by aLexfish.
Before I found these settings, I switched it to modem in (I think) weird way.
I've installed winxp in virtualbox. Modem has drivers for windows. So i thought that i can use it in VB. So I've installed modem in winxp.
After that everytime when I pluged the modem in it logged in as 0af0:d001 (didn't work as a modem) but after starting my virtual winXP i automatically switched as 0af0:d157 in linux and worked as a modem even after closing VB.
Strange isn't it?
Thanks for the new device data!
BTW, the method you described to get your modem working at first is not weird at all; it's the only possibility if you can't find out the exact command which does the mode switch.
The whole point is that the Windows driver always knows how to do the switch; on the other hand the device will stay in the new mode as long as it is powered, independent of the operating system or the driver.
